I've also had it happen when teaching. several times in fact. This last weekend I had a guy having trouble. I stuck the tool into the wood and it wouldn't cut. So I took his tool over to the grinder and got it good and sharp. When I went back to cutting I noticed the switch on the powermatic was in reverse. In fact he hit that switch 2 more times. I had told them from the beginning not to touch that switch.

Yeah, that's a clasic....

It can even happen on the bandsaw. If you are replacing blades, if you are a little "exuberant" when uncoiling the blade, it can open inside out. If you install it with the teeth pointing forward (nobody would make the mistake implied there) they are pointing up. You'd probably notice it with a 3 tpi resaw blade, but a 6 or 10 tpi blade might just escape your notice. I've even seen people mount a table saw or radial arm saw blade backwards.

That has never happened to me! Honest! Can't afford a lathe with reverse so I just walk around to the back side and do my "reverse" work from there. Works great.

Oh, I've done it on a much larger piece. But, with a much larger piece, my lathe spins up more slowly and I can see that lathe is spinning in reverse. With this small piece, by the time my eyes moved from the switch to the piece, it was already spinning too fast to see the direction of rotation.

I'm just making excuses. I was too tired to be using power machinery! I should have called it a day long before that, but I wanted to finish the project!
