No6 Follow up

Thanks for again for all the help with the frog/yoke problem I'm having with this old No6 fore plane. I took some time tonight and punched the pin out so I could measure it. While I was at it, I took some photos. A couple show the yoke and pin (pin is 0.153 dia x 0.685 lg), while others show the frog assembly and body.
You can't tell it from the photos, but the yoke appears distorted. Almost like it was bent to one side. Couple that with the chipped portion that is received by the ship breaker, and I think I might have a cause of my problems. You can sort of see the warp when it's viewed alongside the #13 Starett Double Square I picked up from Walt last week (drive by gloat!!)
I'd also like to see if I could get some help identifying this Fore Plane. To be honest, when I purchased it I was too excited to remember what the gent said it was. The body of this Fore Plane has the number "3015" cast into it. And the base of the frog has the number "418" cast into that. Any help here?
