Dieter Schmidt sells a tap and screw box for 62 mm shaft for about 1200 Euros. My bench screw was made from dogwood using these tools in 1977. The quality is very fine, much nicer than a router made screw.

For making one or two screws it is more economical to just carve a screw. It is surprisingly easy to make something uniform and functional with a chisel, maybe trued with a file.

Making the nut is more involved. There were wooden taps in the Dominy tool collection, probably used to make their double screw vises. Here is an excellent video showing a wooden tap at work. Both these methods are slower than using tools like the schmidt screwbox, but reasonable for small quantities.
