One contractor."},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><strong>UnitedHealth Group (via its Optum subsidiaries) sat on both sides of buprenorphine\/Suboxone issues: helping evaluate its safety for the VA while also supporting marketing of the product for the manufacturer, with dental harms allegedly omitted from VA guidance.<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Corporate ownership (\u201cSame Umbrella\u201d)<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>The Lewin Group<\/strong> is a long-standing health policy research\/consulting firm acquired by Ingenix (a UnitedHealth subsidiary) and later folded into Optum. It maintains claims of analytical independence but is owned by UnitedHealth Group.<\/li>\n\n\n\n<li><strong>OptumInsight<\/strong> is Optum\u2019s data, analytics, research, consulting, and technology arm (also under UnitedHealth). Legacy entities feeding into it include Ingenix and the Lewin Group. Optum as a whole is one of UnitedHealth\u2019s major non-insurance businesses.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This ownership overlap is accurate and well-documented.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The two contracts\/sides of the claim<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Left folder (VA Evidence Synthesis Contract \u2013 The Lewin Group \/ Department of Veterans Affairs)<\/strong>: The Lewin Group has held VA contracts related to evidence-based clinical practice guidelines. It facilitated meetings, key-question development (using PICOTS frameworks), and related work for VA\/DoD guidelines, including the 2021 Management of Substance Use Disorders guideline (which strongly supports buprenorphine for opioid use disorder) and aspects of opioid\/chronic-pain guidance that discuss or prefer buprenorphine in certain contexts.<\/li>\n\n\n\n<li><strong>Right folder (Suboxone Marketing Support \u2013 OptumInsight \/ Reckitt Benckiser \/ Indivior)<\/strong>: OptumInsight and related Optum entities have been alleged in opioid litigation (including MDL-related matters) to have partnered with manufacturers on studies, analytics, marketing support, educational programs, and related work. The image specifically ties OptumInsight to Indivior\/Reckitt support for marketing Suboxone\u2019s safety profile. Broader Optum\/OptumInsight involvement with opioid manufacturers (studies, data, formulary\/rebate dynamics, etc.) appears in various state and MDL complaints.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Indivior \/ Reckitt settlements and deceptive marketing<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Reckitt Benckiser (Indivior\u2019s former parent) paid approximately <strong>$1.4 billion<\/strong> in 2019 to resolve federal criminal and civil liability related to Suboxone marketing. Indivior later resolved additional criminal and civil matters (hundreds of millions more; total resolutions related to the marketing conduct exceeded $2 billion in some accounts). Conduct included false or misleading claims about Suboxone Film\u2019s relative safety (especially regarding accidental pediatric exposure) used to promote switching from tablets, delay generics, and support coverage\/formulary placement, plus other improper promotion and related practices.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Dental harms from Suboxone \/ sublingual buprenorphine<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Sublingual\/buccal buprenorphine products (including Suboxone film and tablets) are acidic. Prolonged contact in the mouth has been linked to serious dental problems: tooth decay\/cavities, erosion, infections, fractures, and tooth loss\u2014even in patients with no prior dental history. The FDA issued a Drug Safety Communication in January 2022 warning about these risks, requiring label updates, and noting hundreds of reported cases (many serious).
