I'm with the loose set screw crowd because mine is...
Not sure what manufacturer, but I have found two set screws in the same hole, one to hold the pulley to the shaft and the other, as far as I can tell, as a jamb set screw, if that makes sense. I've seen this on more than one occasion but couldn't tell if it came from the factory that way.
