Usually too much pressure. To much on the bevel can do this, too much on the cutting edge too is a problem.

I prefer to hold one end in a chuck. This makes the entire spindle stiffer and can eliminate a lot of vibration from flexing on smaller spindles. I don't know about your mallet but still I'd use a chuck on a short tenon.

Once you get the existing spiral cleaned up a lighter touch will probably give a very nice surface. Sharp edge, of course!
