Absolutely! He did lots of things wrong, and that was one of the biggies. Plywood, even high-quality birch or apple ply can delaminate.

He also didn't spread the glue across the entire surface of the wood when he was gluing up the layers or when he glued on the glue block. I'm sure some of the layers had dry spots in the glue joint. If you're gluing up so many layers, and then turning them down to a fairly thin wall, any gaps in the glue could cause huge problems. I don't know if that contributed to the failure, but it sure could have.
