I would use a chuck with pin jaws to hold the wood stock. I would use a 3/8" spindle detail gouge. If I understand you correctly, you want to turn the knob with the face of the knob being face grain, so you can carve it. Not knowing what type of wood you plan to use, but the knob will be weak at its narrowest cross-section with the wood oriented like this. I live in Knoxville area, if you want some help.
