Failure can be a combination of things. Most common are from going too fast and too hard. Slow drill speeds, 600rpm and less work best, and no more than the weight of your drill. With extended use, they do wear out. I one mandrill, and a interface pad which is cheap to replace. I prefer the firm pads from Vince that have the 1/4 round profile on the edge up to 180 or 220, and go with the medium pad up to 400. I seldom go over 400, but past that I would use the soft pads.
