Not familiar with that saw.

Is this a new problem or has this been a problem for a while?

- Does this happen while you are trying to tension?
or
- after the chain is properly tensioned and while the saw is cutting?

I can't imagine the tension slipping while the saw is running since at that point the bar should be clamped tightly to the saw by two nuts or other means and the tensioner should be doing nothing (if it's like other saws I'm familiar with).

Does the "piece on the end" or the edge of the hole in the bar show wear?

Maybe describe in more detail.
