Knots and really wild grain, maybe something else. Sometimes a stray grit on a stone ("clicking is nicking!"). But mostly knots - they can take quite a chunk from a blade. After that there will be lines or even narrow stripes on a finished surface, can be seen and felt. You probably care about it too regardless sanding vs finish planing debates — the whole cutting edge will fail very soon after the first nick.
