Wearing uses treaded rods of various diameters, mostly 5/8", then M10 and M12, depending on an application. On a few occasions he also recommends threading a rod yourself. The dovetailing vise project suggests using M12 thread.

He also uses pretty regular turned handles like F-clamps or double screw have. As Bill has already mentioned, this type of device doesn't really need lots of torque on the shafts, so these handles are more than adequate.
