What is the actual thickness of the planed 5/4? And what is the intended final thickness of the finished table top? I ask to get at how much leeway you have to do further stock prep. This gets at questions like whether to glue up first, before final flattening and grain-taming. And whether to try to work with that twisted board, or whether to rip it in two to minimize the planing loss, or buy a new straight board, etc.

And whether to sand the surface to deal with the tearout, as has been mentioned already. This is heretical of course, but if you don’t have much leeway, might be the practical thing to do. And become a planing expert on waste wood.
