Yeah, a friend who flies some very high-profile clients pointed that report out to me. Apparently the accident pilots have achieved a certain degree of posthumous notoriety within that community. The crash was news at the time because it killed Lewis Katz (see link).

My friend's take is that the pilots had decided that they were so good at what they did that administratrivia such as checklists needn't apply to them. Even with all of that they could have saved themselves if they'd rejected the takeoff as soon as they realized what was wrong, but they apparently feared losing their licenses in the ensuing FAA investigation more than death.
