Maintaining a round tool rest?

I'm not understanding the way to handle nicks and dings caused by catches. It seems if you file the nick flat you will create a dip in the round bar. Do you file the entire length of the bar when taking out the nick? Wouldn't this then cause a flat in the bar and then another when you repair the next one. If this is the case after a bit you would have a pretty beat up tool rest. I'm fairly certain this isn't case but would like to know how they are handled.
