Try some drawings - maybe you can come up with a good solution, but I haven't been able to do it. (Not that I've tried that much.) Wally's example and other nice collared pieces that I've seen approach the horizontal as they approach the entry hole. I don't think you want them to be exactly horizontal (in general), but maybe up to 10 or so degrees from horizontal.
