Everyone should take away from this the caution to examine the piece of wood and learn to recognize fatal defects in the piece before they put it on the lathe. He saw the bark inclusion in the crotch, and should have recognized that his piece was actually two, held together by bark and a bit of sap wood.

A wood feather and some curly grain ain't worth dying for, folks. Consider also that if that piece had been spinning a bit faster and if he was in position to true up the rim, that light weight flip-up face shield would not have saved his life.
