Not every time, I guarantee it since mine was NOT set screws. I, and the PM repair guy, independently ruled out everything else (set screws, pulley alignment, shaft rotation sensor, etc) and isolated it to the motor under side load from the belt. Replaced the motor as mentioned in previous reply and ended up with the same problem shortly on the new motor.
